Guided Instructions for each form

1.  ABC Observation

This form is completed by the classroom teacher when a child continues to demonstrate challenging behaviors after universal teaching strategies have been used consistently for at least 5 school days. Be specific and objective. Do not use labels or diagnoses. Submit all forms

2.  Behavior Support Referral

This form is completed by the teacher after completing and submitting all ABC data forms. Record only what is seen and heard. Avoid assumptions or interpretations. Multiple observations may be completed for the same child to identify patterns.

 

3. Parent Consent

This form is reviewed with the parent/guardian before in-house behavioral supports begin. Administration is responsible for explaining the purpose of the support and obtaining consent. A signed copy must be kept in the child’s file.

4.  Individual Behavior Support Plan

This plan is developed based on observation, data collection, and collaboration between classroom staff and the Behavior Support Lead (BSL). Strategies must be realistic, clearly defined, and consistently implemented by all staff. The plan will be reviewed with families for alignment and formally reviewed by teachers and the BSL every 2 weeks, or sooner if behaviors escalate. Progress will be monitored through ongoing data collection to guide any needed adjustments.

 

5.  Coaching & Implementation Log

This form documents Coaching  short-term and focuses on building staff independence in implementing behavior support strategies. Each case will include an initial coaching cycle of approximately 2–4 weeks, The number of coaching sessions will be determined based on staff implementation and child progress. Most cases will not exceed 4–6 coaching sessions unless additional support is needed. Ongoing decisions will be guided by data and implementation fidelity to ensure effective and sustainable outcomes.
